Young U.S. Women: New Face Of Heroin Use

Heroin use in the United States (U.S) like in other parts of the world has been predominantly men. The few women found in this category have been those in the inner city, rich and spoilt. However this is fast changing. Within this past decade heroin use has hit a staggering 62%. In 2005 heroin use was reportedly slightly over 1.6% in every 1000 Americans, aged 12 and over. In 2015 however the percentage went up somewhere above 2.6% per a similar number in U.S.

The rate of heroin use doubled that of men in a similar period. According to a report by the Centers for Disease Control and Prevention (CDC), the percentage gradually increased from a mere 0.8% back in the year 2002 to 1.6% women alone. According to the report, the face of heroin use has changed from a stern faced man in the city unexpectedly to a young beautiful woman in the suburb. The report also indicated heroin-related deaths have hit of 300 percent.

Increasing Heroin Use in Women a Result of Pain Killer Prescriptions

The report pointed out different groups in the U.S. that have never been known to use heroin have been lured into the same in more recent times. It indicates a leading group of increasingly heroin users are women, those with higher incomes and private insurance. Form Heroin Task Force to Fight Increasing Use

Various authorities argue the war on heroin use has been in existence for some time, with a few successes however it has continued to increase. In La Crosse County, the officials noticed heroin use was getting out of hand and decided to face it head-on. They formed the little known La Crosse County Heroin Task Force.

“We noticed heroin use had increased as early as the year 2010-2011,” said Al Bliss, then the task force coordinator. However he noted even at that, a lot more still has to be done. “I think we’ve made some end roads at decreasing availability of heroin but it still remains a problem,” added Bliss.

The task force coordinator also said in order to curb the menacing heroin problem, communities needed to form programs more or less similar to the Heroin Task Force so as to get like-minded people working together at attaining a single determined goal. “We need to further educate the community in offering help, treatment, and preventive care; and to conduct a lot of awareness in order to address the comprehensive problem of drug abuse,” he said.

In a separate interview, Dr. Akoury echoed Mr. Bliss’s earlier observations; she said people suffering from substance abuse are more likely to form a habit of popping in pills. She added they are more susceptible to heroin use. A number of studies and medical reports also support this claim. One commonly cited study indicates alcohol and cigarette smokers have a higher chance of about 5 times their fellows who do not. The CDC report actually indicated 96% of heroin users used at least one more addictive drug. Also, that 61% of the heroin users used at least three more addictive substances.

In conclusion Dr. Frieden, director of CDC empathized with the current state of heroin use. He said: “It is heartbreaking to see injection drug use making a comeback in the U.S.” He said solving the heroin issue boils down to preventing addiction in the first place by hunting down the primary cause, which according to him were opioid prescriptions. “It also means; increasing access to rehabilitation including medications such as methadone or suboxone, cracking down on heroin sales, and increasing use of naloxone to reverse overdose. These are the traditional basics to deal with addiction, hence also key in combatting heroin use,” quickly added the CDC director

Gluten Free Diet for Fibromyalgia

Gluten Free Diet is good Fibromyalgia patients

Gluten free dietFibromyalgia is a disorder characterized by widespread muscle pain, joint stiffness, and fatigue. The condition is chronic but pain comes and goes and moves about the body. The cause of this disorder is unknown and therefore it is often misdiagnosed or unrecognized and is most of the times complicated by anxiety and mood disorders. this is a common disorder that according to the American College of Rheumatology diagnosis criteria affects about 3-5% of women, most of whom are between ages 20 and 50, but only 0.5- 1.6% of men. This disease is more common in adults than in children but it affects women more than men.

What is gluten

Gluten is the protein in wheat, barley, rye, spelt, Kamut and triticale. Just as any other foods there are people who are allergic or tolerant to gluten. It is suggested that for these people removing gluten from their diets can help them as it may lead to improved skin, including fewer problems with breakouts, eczema and psoriasis; fewer migraines; and a lessening of vague, general symptoms of malady. Gluten has been linked to fibromyalgia and now it is suggested that those who are suffering from fibromyalgia can benefit from a gluten free diet.

Fibromyalgia is characterized by chronic pain sleep disturbance and fatigue combined with a general increase in medical symptoms such as problems of memory and often psychological distress. The pharmacological treatment of Fibromyalgia does not result in full healing but only offers partial relief. Fibromyalgia is mostly associated with depression and irritable bowel syndrome. On the other hand, Celiac disease is a frequent disease, affecting about 1 % of the population that can be diagnosed at all ages under different clinical settings. The classic presentation of Celiac disease is chronic diarrhea, positive anti-tissue transglutaminase antibodies (anti-tTG), and villous atrophy with intraepithelial lymphocytosis observed by duodenal biopsy. Celiac disease often does not conform to the classical clinical description, and up to 50 % of patients with adult Celiac disease lack prominent gastrointestinal symptoms. Clinical manifestations of Celiac disease include muscle and osteoarticular pain, fatigue, and neurological and psychological symptoms.

Research report on gluten-free diet

In a research that was aimed to describe selected patients with fibromyalgia, non-celiac gluten sensitivity, duodenal intraepithelial lymphocytosis, and clinical response to gluten-free diet, in order to raise the hypothesis that non-celiac gluten sensitivity could be a treatable cause of fibromyalgia it was found that remarkable clinical improvement can be achieved with a gluten-free diet in patients with fibromyalgia.

This study involved a trial of a strict gluten-free diet that was conducted with the help and training of the Celiac Disease Patients Association of Madrid. In addition, iron, vitamin D, and multivitamins supplements with oligoelements were prescribed to patients on this study. A lactose-free diet was also recommended when lactose intolerance was suspected on clinical grounds or demonstrated by a breath hydrogen test after lactose administration. Clinical response was defined as the achievement of at least one of the following scenarios: remission of fibromyalgia pain criteria, return to work, return to normal life as judged by the patient, or opioid discontinuation.

For this study, the mean follow-up period for the gluten-free diet was 16.4 months .Eight patients were also on a lactose-free diet. For five of these eight patients, a lactose-free diet had been attempted before the gluten-free diet, resulting in partial relief of the gastrointestinal symptoms but no improvement in the fibromyalgia symptoms. For three patients, a lactose-free diet was started concurrently with the gluten-free diet. The level of widespread chronic pain improved dramatically for all patients; for 15 patients, chronic widespread pain was no longer present, indicating remission of fibromyalgia. Fifteen patients returned to work or normal life after good improvements with the gluten free diet. In three patients who had been previously treated in pain units with opioids, these drugs were discontinued. Fatigue, gastrointestinal symptoms, migraine, and depression also improved together with pain. The clinical improvement for some patients began just a few months after beginning the gluten free diet therapy this was striking and showed how effective the gluten free diet therapy is beneficial to those suffering from fibromyalgia. For others the improvement was rather slow and gradual and took many months of follow up. It was also seen that some patients’ conditioned worsened when they took gluten but this was save when they were put on strict-gluten free diet.

Gluten Free dietIn this research it was observed that the reduction in the level of pain was accompanied by improvements in asthenia and gastrointestinal and neurological symptoms. This shows that a common underlying cause of fibromyalgia is related to gluten.

For this research, the clinical response definition was decided upon after the initial observations of impressive improvement in some patients after starting the gluten-free diet. This was the chosen outcome measure; the remission of fibromyalgia or return to normal life, rather than changes reflected by questionnaires. This is one of the treatments that have shown the highest degree of improvement in treatment of fibromyalgia. Most of the patients that were used for this research were under severe distress and disability and were unable to cope normally with daily activities or on sick leave, despite having been treated with many different regimens for years. This fact endorses the fact that gluten-free diet can help patients suffering from chronic pain as a result of fibromyalgia.

In 1986, Congress passed the Emergency Medical Treatment and Active Labor Act (EMTALA) (42 U.S.C.A. § 1395dd), which establishedcriteria for emergency services and criteria for safe transfer of patients between hospitals. This statute was designed to prevent transferring an undesirable patient to another facility.The law applies to all hospitals receiving federal funds, such asMedicare. The law requires hospitals to provide a screening exam to determine if an emergency condition exists, providestabilizing treatment to any emergency patient or to any woman in active labor before transfer, and continue treatment until a patient can bedischarged or transferred without harm. It also delineates strict guidelines for the transfer of a patient who cannot be stabilized. A hospitalthat negligently or knowingly and willfully violates any of these provisions can be terminated or suspended from Medicare.The physician,the hospital, or both can also be penalized up to $50,000 for each knowing violation of thelaw.

Addiction TreatmentAny patient who needs emergency attention should receive it since if it is delayed there might be a loss of life. Therefore if a doctor is asked to attend to a medical emergency as defined in this statement, he must respond. This is both an ethical and legal obligation and therefore every doctor should abide. However rare there will be times when attending a medical emergency is impossible or unsafe for the doctor or patient. If a doctor chooses not to attend he or she may be required to defend that decision in the event of a charge of professional misconduct or criminal prosecution. The patient who needs medical attention is therefore catered for and should not be ignored.

A doctor is at risk of being professionally or criminally responsible of the death of a patient if he or she fails to render prompt and appropriate medical care to any person whether the patient is a current patient has just came in, in a medical emergency. A doctor who chooses not to attend must have good reason and be able to defend this position at a later time otherwise he will be charged in a court of law for medical negligence.

However there are situations when a doctor, may or should not attend a medical emergency. These scenarios may include;

  • if he or she is already attending another emergency
  • if it is more appropriate for an emergency service to attend (i.e. ambulance or rescue helicopter);
  • the geographical location of the doctor is such that another doctor or medical service can attend more promptly;
  • if he or she is off duty at the time of the call and has been drinking alcohol or taken medication or other substances to a level that may adversely influence the doctor’s level of competence;
  • If attending the emergency places the personal safety of the doctor at risk.
  • The doctor may also not attend to the patient if he is not in the right state of health himself or when he knows he is not competent enough to help the patient.

However a doctor should be able to take good care of the patient even if he is not able to offer any reasonable help to the patient. He should be able to assist the patient to access another health center where he can be helped. If a doctor fails to attend to emergency because of matters of inconvenience he will be charged and prosecuted.
